Use the simple steps below: 

  1. Go to https://signup.formed.org/ to sign up.
  2. Enter “St Helen School” and select the entry with the church address (3894 Triumph St Burnaby BC )
  3. Click Next
  4. Enter your name and email address. Click Sign Up

You’re in! After you Sign Up, download the FORMED App by searching “FORMED Catholic” in your App store.
